The patient who has irreversible renal failure should be treated by dialysis or renal transplantation. Glomerulonephritis and diabetic nephropathy are the major cause of irreversible renal failure.
The mechanism of renal injury has mainly two ways. One is that immune complex attaches to glomerular basement membrane, which is the major cause of glomerulonephritis. The other is that polyanion and foot process of glomerulus are disappeared, which is the major cause of diabetic nephropathy.
If glomerular injury is occured, the protein or red blood cell can go out from glomerulus to urine. When one has glomerular injury we can find out proteinuria, hematuria, edema or hypertension from that patient. Heavy proteinuria makes edema, which is named nephrotic syndrome. The mechanism of edema in nephrotic syndrome is that proteinuria makes hypoproteinemia in blood, and so the osmolarity of blood is reduced. The water in blood goes to the interstitium, Which is edema. Therefore the edema of nephrotic syndrome can be treated by the control of proteinuria.
So I deviced the astriction-urination theory for the treatment of edema by the control of proteinuria.
With the view of oriental medicine we think the edema is occured by the loss of normal function of the spleen(9*), the lung(N) and the kidney(*) and as the result the water in blood is stored in body. Nei Ching(MV) suggested the theory of sweating, urination and severe urination to treat the edema.
Doctor Chang (N) spoke that the edema in the upper body can be treated by sweating and the edema in the lower body can be treated by urination. Doctor Choo(*) thought helping spleen, circulating water and urination as the main method of treating edema. Doctor Lee distinguished positive edema by outer factor from negative edema by inner factor and he thought that the treatment - of positive edema is sweating and urination and the treatment of -negative edema is helping spleen and circulating water.
Recently Doctor Chang and some other doctors researched various former theory of treating edema and classified fourteen methods of treating edema. They are urination, sweating, severe urination and so on. But I couldn´t.. -find the astriction--urination anywhere in their research.
I added Nelumb Semen(X-f-A), Maximowicziae Fructus(K*-T), Dioscoreae Radix(dj ), Mastodi Fossilia Ussis(Akt), Ostreae TestaWIXV) and Mantidis Vagina OvorumMOM) which have astricting effect to Soowhabooncheongyeum(* Aftxfk) that treat cloudy urine. I called it Gamisoowhabooncheongyeum(n~t,-*A 5} g fx) and it has the effect of astriction-urination.
I treated 5 patients who have edema, hematuria or proteinuria after glomerular injury with Gamisoowhabooncheongyeum(ba*, cA53´i~mpkatlkk) and the result of treatment was satisfied.
The results obtained in this research were as follows :
1. Astriction-urination theory was effective to treat the patients who have glomerular injury with proteinuria and hematuria.
2. Gamisoowhabooncheongyeum restored the hematuria resulted from the glomerular injury to the normal range.
3. Gamisoowhabooncheongyeum improved the proteinuria resulted from the glomerular injury.
